How to: Remove a Page header/footer from a form.
Solution:
In the Form Design view, select the 'View' menu and select 'Form Header/Footer'.
NOTE: Page headers and footers CANNOT be removed separately, that is, just a page header cannot be removed, or just a page footer cannot be removed. However, the undesired header or footer can be hidden.
1) If the Database window is not active, activate the Database window.
2) Click the 'Forms' tab in the Database window.

3) Click 'Design'.
4) Select the 'View' menu and select 'Page Header/Footer'.
NOTE: If the Page Header/Footer contains controls, a prompt appears to verify the removal of the header/footer and all the controls.
WARNING: Choosing the 'Yes' in the prompt permanently deletes the controls within the header and footer.
Select Page Header/Footer.
